This page provides useful employment information, links and commonly used forms, applications and agreements used at the Kansas State School for the Blind.

    An alternate method of accessing the employment forms linked in this page, is to go to the bottom of this page for the individually listed files under the heading Attachment.



    This PAYROLL SECTION contains commonly used forms, applications and agreements used to process payroll.

    • State of Kansas Direct Deposit form authorizes the direct deposit of your paycheck. This optional method of payment is for regular KSSB employees, but is not available for short-term temporary appointments (i.e. summer school session).     • KSSB Employee Personal Information Form updates administration of telephone number, mailing or email address changes.
    • KSSB Timesheet is required to process hours worked for FLSA non-exempt (hourly) appointments. Timesheets must have required signatures in order to be processed.
    • KSSB Leave Application is required to process approved leave for benefit eligible appointments. Leave Applications must have required signatures in order to be processed.

  • The following link provides information on Health Insurance benefits. Health insurance benefits are administered by the Kansas Health Policy Authority. Health insurance benefits can be selected upon initial employment and will commence after completing an approximate 60 day waiting period, or selected during the annual "open enrollment period" which occurs every October with an effective date of January 1st.   • The following link provides information on the HealthQuest Employee Assistance Program (EAP) for State of Kansas employees. This confidential assistance is available 24 hours a day at no initial cost to you, just call the HealthQuest Lifeline number, 1-800-284-7575. A single call to the HealthQuest Lifeline will provide you and your family members help on a variety of issues, such as short-term counseling, family resources, elder care information, legal advice, personal money management, life coaching, and healthy weight classes. To learn more, go to the HealthQuest Employee Assistance Program website administered by the Kansas Health Policy Authority.
  • The following link provides information and claim forms on Flexible Spending Accounts. The Flexible Spending Accounts vendor is ASIFlex. This optional benefits allows eligible employees to pay for qualified unreimbursed health care and dependent day care expenses with pretax earnings deposited into special employee accounts. Go to the Flexible Spending Accounts website managed by ASIFlex.
  • The following link provides information on primary Retirement benefits as a defined benefit pension plan. This primary retirement benefit is administered by the Kansas Public Employees Retirement System (KPERS). Participation in this retirement plan is mandatory for all eligible employees. Go to the Kansas Public Employees Retirement System website.
  • The following link provides information on an optional retirement benefit as a Deferred Compensation Plan (Section 457 of Internal Revenue Code). This additional retirement benefit is administered by ING. Participation in this retirement plan is voluntary for all eligible employees. Go to the Kansas Public Employees Deferred Compensation Plan website.
  • There is another optional retirement benefit as a Voluntary Tax Sheltered Annuities Plan (Section 403(b) of Internal Revenue Code). This additional retirement benefit allows individual retirement accounts to be established between an employee and approved annuity provider. To enroll in this plan, complete the KSSB 403(b) Salary Reduction Agreement Form which authorizes pre-tax deduction of funds. Participation in this retirement plan is voluntary for all eligible employees.
  • The following link provides information on Life Insurance and Death Benefits. This basic group life insurance benefit is 150% of annual rate of compensation. This benefit is administered by the Kansas Public Employees Retirement System (KPERS). Participation in this benefit is automatic and FREE for all eligible employees.   • The following link provides information on an Optional Group Life Insurance benefit. This optional life insurance benefit is administered by the Minnesota Life Insurance Company. Participation in this additional life insurance benefit is voluntary for all eligible employees. Go to the Kansas Public Employees Retirement System - Optional Group Life Insurance website.
  • The Purpose of FMLA document explains the purpose, eligibility and provisions of the Family Medical Leave Act (FMLA). To initiate the FMLA benefit, an employee needs to complete a KSSB FMLA Employee Request Form as soon as a qualifying event is known. The employee shall be responsible in having the Federal Certification of Health Care Provider forms completed by a health care provider. All completed documents must returned to the Human Resources Department in a timely manner in order to fully utilize this benefit.
  • KS Shared Leave Request Form is for an employee requesting approval to receive donations of leave under the Shared Leave Program for serious, extreme or life-threatening illnesses, injuries, impairments or physical or mental conditions. This benefit is FREE for all eligible employees, but is subject to approval by the KS Share Leave Committee. To learn more, go to the Shared Leave Program website administered by the Kansas Department of Administration.
  • Kansas Workers Compensation Report of Injury Form and Injured Employee's Report of Injury Form are required to report and process Workers Compensation benefits. Requests for qualified mileage reimbursement require the completion of a Mileage Reimbursement Form. Forms must be completed in full and have required signatures in order to be processed.
